The initial consultation is the first step to the LifeLong Mentor process.  The information you provide us will assist in matching you with the best peer supporter, designed to maximize the therapeutic effect of your peer support experience. Once we get to know you a bit better, we will match you with a peer support volunteer we think will be the best suited for your individualized needs. During an initial peer supporter meet and greet, if you are not satisfied with your first matched peer supporter, you can always request for someone else.

Your initial consultation will ask some personal questions and it is important to remember that your form is held in strict confidentiality and privacy.
